Condition Management
Care and Support Empowers Participants to Better Health
Sometimes managing your health requires more than just doctor visits, lab tests and prescriptions. As a member of the Texas Association of Counties Health and Employee Benefits Pool (TAC HEBP), you have access to programs and services, through Blue Care Connection®,designed with your health and wellness in mind. Through phone call outreach and online tools, available programs can help you monitor a health condition, manage your weight, control stress and eliminate tobacco.
Condition Management
If you are at risk for or already have a chronic condition, this voluntary program can provide education, coaching and can help guide you through the often complex health care system so you can focus on what matters most – getting healthy and staying well. Through regularly scheduled health counseling and coaching telephone calls, Blue Care® Advisors, registered nurses or other health care professionals, can help you identify unhealthy behaviors, set wellness goals, adopt healthier habits and learn to manage medical conditions more effectively.
This voluntary health improvement program can help members with: cancer, congestive heart failure, coronary artery disease, chronic obstructive pulmonary disease, asthma, diabetes, metabolic syndrome (high cholesterol, high blood pressure and obesity) and low back pain.
24-Hour NurseLine
Once signed up, you can call (866) 412-8795 anytime to speak with a registered nurse and get answers to your health questions when you need them.
Well onTarget
Designed to give you the tools and support you need while rewarding you with Life Points for making healthy choices. When you enroll in the fitness program or take your health assessment, you earn points. You can also earn points when you achieve milestones in the self-directed courses. Redeem your Life Points for a large selection of rewards in the online shopping mall.

Special Beginnings®
Expectant mothers can manage and understand pregnancy with helpful educational materials and support, pregnancy risk factor identification, and engage in ongoing communication with an obstetric nurse. Members are supported from early pregnancy through six weeks after delivery.
Case Management
Case management assistance is available if you experience a serious illness or injury. Registered nurse case managers can help you or a family member cope with the situation, understand your benefits and identify the right health care resources.
Tobacco Cessation
Want to stop smoking? The Tobacco Cessation Program offered by BCBSTX can help with tools, coaching and support. Plus, enroll in Tobacco Cessation and take advantage of our Tobacco Cessation Prescription Benefit.
Weight Management Program
Get guidance and support for healthy eating habits and weight loss. Set goals, create action plans, and get coaching to stay motivated.
Lifestyle and Wellness Coaching
Receive support from professionally certified coaches who can guide you through the process of making lifestyle changes to improve nutrition, increase physical fitness, manage stress and more.
Blue Access for Members
For personalized information about your health care benefits and coverage, visit the TAC Health and Employee Benefits Pool online portal, select “Get Connected,” then select “Blue Cross and Blue Shield of Texas,” where you'll find:
- Confirmation of when claims are paid and payment amounts;
- Physician and hospital directories (doctors, dentists, hospitals, etc.) with Provider Finder; and
- Ability to track your medical history (doctor visits, illnesses, and prescriptions).
